We are recruiting for two roles: Family Physician and Lead Family Physician. All applicants will be considered for the Family Physician position. FAMILY PHYSICIAN
Salary Range: $166,935 – $172,778

About the Role
As a key member of our integrated health team, you’ll provide high-quality, compassionate care to children, adults, and pregnant women across the Project HOME community—including residents, neighbors, and individuals experiencing homelessness. This role offers the unique opportunity to combine clinical expertise with purpose-driven service in a supportive, multidisciplinary environment.
What You’ll Do
- Deliver comprehensive primary care and women’s health services, including OB and LARC procedures.
- Provide continuity of care through collaborative practice with behavioral health and social service teams.
- Educate patients on preventive health and wellness while addressing social determinants of health.
- Teach and precept medical students and residents, sharing your passion for community medicine.
- Participate in quality improvement initiatives and support Patient-Centered Medical Home (PCMH) standards.
- Contribute to program development that expands access and equity in healthcare delivery.
What You Bring
- MD or DO with completion of a Family Medicine residency; Board Certified or Board Eligible.
- CPR certification (required).
- Experience in primary care and/or underserved populations preferred.
- Proficiency in obstetrics and gynecology services is a strong plus.
- Excellent communication, teamwork, and organizational skills.

LEAD FAMILY PHYSICIAN
Salary Range: $176,473 - $182,650
Job Summary: The Lead Physician is a Physician who is responsible for providing clinical supervision to a team of medical providers and one psychiatric certified registered nurse practitioner. The team includes clinicians who are directly employed or contracted to provide services on-site and through written referral agreements. This process is facilitated by engaging in and promoting regular leadership "rounding" in a way that fosters a culture of open communication, continuous improvement, and patient-centered care. This physician will need experience providing care and supervision for full-spectrum primary care, including pediatrics, obstetrical care, and gynecological care.

WHAT EXPERIENCE YOU MAY BRING:
The ideal candidate will bring extensive knowledge, training, skills, and experience in a multidisciplinary approach to physical health, mental health, and chemical dependency. Leadership experience within an integrated health organization is essential. Proven expertise in treating patients living with HIV, providing prenatal and postpartum care, and delivering inclusive care to the LGBTQ+ community is required. The candidate should be skilled in prescribing medications for opioid use disorder and possess a strong background in continuous quality improvement and leadership development. Experience delivering and overseeing services in diverse work environments, serving vulnerable populations, is highly valued. Familiarity with de-escalation methods-or a willingness to be trained-is expected. A demonstrated commitment to social justice and racial equity, along with excellent written and verbal communication skills, is crucial. Proficiency in using eClinicalWorks is also required.
Essential Functions of the Lead Physician:
The Lead Physician provides direct patient care at Project HOME and plays a critical role in ensuring high-quality, compassionate, and patient-centered medical services. Key responsibilities include delivering comprehensive medical care, including diagnosis, treatment, and preventive care, in consultation with and under the supervision of family physicians. The Lead Physician conducts physical examinations, orders diagnostic tests, administers vaccinations, prescribes medications, and educates patients on health maintenance and disease prevention.
Maintaining accurate and timely medical records, collaborating with other healthcare professionals for integrated care, and adhering to FQHC regulations are essential duties. The Lead Physician participates in quality improvement initiatives and peer reviews and provides clinical supervision in obstetrics, gynecology, and high-risk pediatric cases, in collaboration with the reproductive and child health nurse.
This role requires working both independently and as part of a multidisciplinary team. Meeting productivity expectations outlined in the Project HOME budget and achieving quality improvement targets set in the organization's annual goals are necessary. Participation in the PHHS on-call and evening hours schedule is also required.
Administrative and Supervisory Roles and Responsibilities:
- Provide supervision and direction to all Stephen Klein Wellness Center providers, including medical providers and the psychiatric certified nurse practitioner.
- Conduct regular "rounds" with the clinical provider team to create a culture of accountability while also fostering continuous improvement.
- In collaboration with the Director of Medical Services, ensure staff at all levels have adequate tools to obtain and measure progress toward quality improvement goals.
- Collaborate with the Director of Medical Services and the Clinic Manager to help support providers meet productivity targets.
- Conduct provider/staff recruitment and retention activities
- Perform semi-annual and annual performance evaluations of direct reports; review performance evaluation results of all within their chain of command.
MINIMUM QUALIFICATIONS:
- Currently licensed Doctor of Medicine or Osteopathy in the Commonwealth of Pennsylvania
- Minimum three years' clinical experience working independently in a primary care setting
- Minimum of 1 year of prior supervisory experience
- Must meet Project HOME's privileging requirements as required by the Federal Tort Claims Act (FTCA)
- Current CPR certification or must obtain within one month if not current
Physical requirements: Ability to move to or remain sedentary for periods of time in various locations
